The crosswalk that turns plantar fasciitis into fibromatosis

M72.2 split two ways on October 1. Most crosswalks learned about one.

What Changed

The FY 2027 code counts reconcile. You have seen 190 and you have seen 238. Both are right. CMS counts 190 new reportable codes, 30 deleted and 4 revised. AAPC counts 238 added entries, which includes non-billable headers.

What it does to your claims: the gap is the dangerous part. Of the 30 invalid codes, 15 were deleted outright and 15 became non-billable headers that now demand a child code. A scrubber loaded from an "added codes" list that includes headers will pass one to a payer.

NCCI PTP Version 33.0 has firm dates. CMS issued Change Request 14611 (Transmittal 13946) on September 17. Effective January 1, 2027, implementation January 4. Test files expected around November 2, change reports around November 17.

What it does to your claims: January 1 is a Friday and a holiday. Your first real volume lands Monday, January 4. Six working weeks from the change report.

Denial Watch

2.21%

The share of accounts receivable providers recovered on appeal in H1 2026, down from 2.48%.

Everyone is reporting that denials fell. Kodiak Solutions puts initial denials at 10.63%, down from 12.00%. Fewer outlets are reporting the rest: takebacks rose to 1.57% of accounts receivable from 1.38%, and your appeal win rate dropped.

You are appealing into a lower success rate than last year. The value of a prevented denial went up while you were looking somewhere else.

Coding Desk

M72.2 split two ways, and only one of them looks obvious

  • Plantar fasciitis · FY 2027 codes: M67.A01 right / M67.A02 left / M67.A09 unspecified · Block: M65–M67, synovium and tendon
  • Plantar fascial fibromatosis · FY 2027 codes: M72.21 right / M72.22 left / M72.20 unspecified · Block: M70–M79, where M72.2 lived

Crosswalk M72.2 to M72.20 and you have done the obvious thing. Same category, one more digit, laterality added. It resembles every other FY 2027 specificity expansion your team handled this month.

It is also wrong for most of your volume. Plantar fasciitis is inflammation, and it accounts for the overwhelming share of what carried M72.2. Plantar fascial fibromatosis is Ledderhose disease, a fibrous nodular proliferation. Different condition, different chapter block.

Illustrative scenario. Podiatry office visit, established patient.

Six weeks of right heel pain, worst on first weight-bearing. Point tenderness at the medial calcaneal tubercle on the right. Windlass test positive on the right. No palpable nodules. Left foot asymptomatic. Assessment: plantar fasciitis, right foot.

Correct: M67.A01. The crosswalk trap: M72.21.

Look at what the trap code does. It is valid. It carries correct laterality. It survives NCCI. A payer will likely pay it. And it asserts a nodular fibromatosis that the note rules out in four words when it records no palpable nodules.

That claim is not a denial. It is a takeback holding a reservation.

What this means for you

  • RCM and billing companies - A mis-crosswalked code that pays becomes a client conversation in eighteen months, not a rework item this week. Check the crosswalk once, across every client.
  • Hospitals - Podiatry and orthopedics carry this. A centrally configured crosswalk fails consistently across facilities, which makes the error both easier to find and larger than it looks.
  • ASCs - Low exposure unless you run podiatry. The pattern applies to every FY 2027 split where a parent gained children in two different blocks.
  • Physician practices - A podiatry practice can audit this in an afternoon.

AI in Practice

A crosswalk is a lookup table. It maps an old code to a new one and has no opinion about what the note says.

That gap is the argument for reading documentation instead of mapping codes. CODIO AI separates confirmed from ruled-out and performed from planned, then validates against NCCI, MUE and LCD/NCD coverage while the claim is being built. A code that cannot survive those rules never reaches a payer.

Nothing runs autonomous on day one. Every chart starts in CoPilot review, and the autonomous lane widens only as first-pass accuracy is proven on your charts by your auditors.

One Thing to Try

Pull every claim since October 1 carrying M72.20, M72.21 or M72.22. Check whether the note documents nodules or inflammation. If it says inflammation, you have a paid claim with the wrong condition on it, correctable now instead of defensible in 2028.
