What Are NCCI Edits?
NCCI edits are CMS-published tables defining CPT code combinations that cannot be billed together when performed for the same patient on the same date of service. There are two main types: Procedure-to-Procedure (PTP) edits, which define code pairs where the column 2 code is a component of the column 1 code and cannot be separately billed; and Medically Unlikely Edits (MUEs), which define the maximum units of service a provider can bill for a single CPT code on a single date.
How to Do an NCCI Edit Lookup Manually
Step 1 : Go to the CMS NCCI Policy Manual page (cms.gov) and download the current PTP edit tables — updated quarterly.
Step 2 : Search for the column 1 CPT code (the primary procedure). Review all listed column 2 codes to identify any that appear on your claim.
Step 3 : Check the modifier indicator. A modifier indicator of 0 means the edit cannot be overridden by any modifier. A modifier indicator of 1 means a modifier (e.g. -59, -XU, -XS) may be appended to the column 2 code if the service was distinct and separately identifiable.
Step 4 : Apply the appropriate modifier if clinically justified and documented, or unbundle and bill only the column 1 code.
